Registration Requirements for students entering Miami-Dade County Public Schools.
A parent/guardian must present the following documents in order to register a student
Mondays – Fridays 7:00 a.m. to 11:00 a.m.
Proof of residence needs to be presented by the parent/guardian at the time of registration.
A. ORIGINAL Florida Power & Light receipt showing name and address of parent/guardian. (NO COPIES)
B. ORIGINAL lease agreement that has been notarize
C. ORIGINAL mortgage that has been notarized.
Failure to submit any of the above documents will result in an automatic address verification. Any student not living within the designated boundaries for Miami Central Senior High will be directed to their home school.
A. Florida Department of Health Student Exam (Form DH3040, including proof of tuberculin skin test).
B. Florida Department of Health Certification of Immunization (Form DH 680).
C. Copy of birth certificate or passport.
D. Previous school transcript or last report card.
Student must have transfer, current immunization records, and grades from previous school.
FRAUDULENT DOCUMENTS WILL RESULT IN IMMEDIATE WITHDRAWAL TO HOME SCHOOL.
All Official Transcripts are $2.00 (cash or money order only) and can be paid in person or by mailing a transfer request to Miami Central Senior High. Transcripts may take up to 10 days to come in to our office before they are available for pick up.
